Referral to Special Education

1. What information do I need in order to register my student?

 

2. What is a 504 plan? Do I need one if my child is in special education? A. It is a plan developed to provide modifications for students with disabilities. If your child is in special education, modifications will be developed at the individualized education program (IEP) meeting.

3.    Q.  How do I get a copy of my rights?         A.  You may receive a copy of your rights by contacting the Special Education                Department at the central office or by contacting your local school.                Parental rights   4.    Q.  How do I get my child into special education?         A.  Students are referred to special education through the Student Support Team               process or by way of a direct parent referral. 5.    Q.  How do I make a referral for my child to receive special education services?         A.  Talk to your child's teacher.  A referral can be made to the Student Support Team               through the learning support strategist at your school.   6.    Q.  I have a two year old who has a disability.  When and how can he/she receive               services?         A.  Children become eligible for services through the school district on their third               birthday.  Evaluation of communication, cognitive, motor, self-help and social               behavior skills is provided. Eligibility for services is determined by a team of               professionals, of which you are a member. 7.    Q.   Can I get services for my four year old child?         A.   Cobb County provides services for preschoolers who have disabilities, be-                ginning at age three, who are eligible to receive services.  To make a                referral, contact the Special Education Department at 770-426-3331. 8.    Q.   How long will it take to get my child into special education?         A.   Under state guidelines, the local school system currently has 60 calendar days                from the time a student is referred to the Special Education Department of                the local school, either by the Student Support Team or by direct referral of                the parents.           A.  1) copy of current IEP (Individual Education Plan) if your child is already Special      
                   Education.    
                  2) immunization certificate on a Georgia certificate form (this is available through 
                        the Georgia Health Department)  
                   3)Social Security card or number 
                   4) in the case of divorce or separation, proof of legal  
                        guardianship    
                    5) copy of child's birth certificate
            For Hearing Impaired student:
                6) copy of current audiogram (within one year)
                7) copy of physician's otology report clearing a student for use of a hearing aid or 
                     assistive listening device
            For Vision Impaired student:
                 6) copy of a current eye report from a physician (done within three years) 
                       reporting vision numbers of best corrected vision in both                                      
                       eyes
